Chronic kidney disease (CKD) is a modern day epidemic which has significant morbidity and mortality. It is associated with various ineral and bone disorders which are now collectively referred to as CKD– mineral and bone disorder (MBD). Alteration in vitamin D metabolism is one of the key features of CKD–MBD that has major clinical and research implications.

Vitamin D deficiency is seen since early stages of CKD and has been described to begin even before other abnormalities in mineral bone metabolism such as hyperphosphatemia become detectable.

Even though there is ample biological and observational data to support the importance of vitamin D in CKD, especially in the Bone and mineral metabolism, the size and quality issues of the existing trials limits conclusions that can be drawn regarding their effects on patient-level skeletal outcomes.

Many questions regarding the need, dosing and the preparations of Vitamin D that must be used in CKD are unanswered. Large scale randomised controlled trials with well-defined patient centric outcomes defining the role of Vitamin D in CKD and CKD_MBD are the need of the hour.
